The Dream Request Program assists people with physical challenges
to obtain the necessary equipment to achieve their dream of
independence.

Here are a few examples of what may be needed:

  • medical equipment
  • a job


Please submit your dream request in writing to us outlining the
following to see if you qualify:
  • Your diagnosed physical disability and how it
    affects your daily life

  • Your dream

  • What you hope to accomplish with the dream

  • How this dream will assist you in the future
